Disclaimer
ACKNOWLEDGEMENT OF RISK AND WAIVER OF LIABILITY
I, the undersigned, acknowledge that my participation in any activities, classes, use of equipment, or
presence at ZFit ("the Gym") involves inherent risks, including but not limited to physical injury,
illness (including but not limited to COVID-19 or other communicable diseases), permanent
disability, and death. I freely assume all such risks, both known and unknown.
ASSUMPTION OF RISK
I understand and acknowledge that ZFit may contain weightlifting equipment, cardio machines, and
other fitness-related apparatuses. I also understand that participation in group fitness classes or
personal training carries its own risks.ZFit Liability Waiver and Release Form
I confirm that I am physically capable of participating in fitness activities at ZFit and have either
consulted with a physician or voluntarily decided not to.
RELEASE OF LIABILITY
In consideration for my access to ZFit and its services, I, on behalf of myself, my heirs, executors,
administrators, and assigns, hereby release, waive, and discharge ZFit, its owners, employees,
agents, trainers, and affiliates from any and all liability, claims, demands, actions, or causes of
action arising out of or related to any loss, damage, or injury, including death, that may be sustained
by me or my property.
INDEMNIFICATION
I agree to indemnify and hold harmless ZFit, its officers, employees, and agents from any and all
claims, actions, suits, procedures, costs, expenses, damages, and liabilities brought as a result of
my involvement or presence at ZFit.
SEVERABILITY
If any portion of this waiver is held to be invalid or unenforceable, all remaining provisions shall
continue to be valid and enforceable.
BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THIS
